AZNHA Program Policies & Waivers

The following policies and waivers are in effect for everyone that participates in a Program or Event hosted by the Arizona In-Home Care Association (AZMHA), including attendees, speakers, partners and vendors.  As part of the registration (or RSVP) process, individuals will be required to acknowledge they have read, understand and agree to abide by the policies and are agreeing to the waivers.  Questions about the policies and waivers may be directed to the AZNHA Administrator via email info@aznha.org or phone 602-283-3503.

Waivers

The following Waivers are in effect for all AZNHA Programs.  As a condition of registration and/or participation in an AZNHA-hosted, all registrants (including those that are registered by an AZNHA Team Member upon request) agree to the statements in the waivers. 

Liability Waiver

As a condition of participation in an AZNHA Program, individuals waive any claim they may have against the AZNHA and its officers, directors, employees, or agents, or against the presenters or speakers, for reliance on any information presented and release AZNHA from and against any and all liability for damage or injury that may arise from participation or attendance at the program. Additionally, individuals understand and agree that all property rights in the material presented, including common law copyright, are expressly reserved to the presenter or speaker or to AZNHA. Individuals acknowledge that participation in events and activities brings some risk and participants must assume responsibility for their own well-being. Note: In the case of a substitution as provided for in the AZNHA Substitution Policy, the new attendee automatically agrees to this disclaimer and waiver by default of the transfer.

Media Waiver (Photo Release)

Individuals understand and agree that there is NOT a reasonable expectation of privacy connected to their physical participation in the program, including their digital presence (face or voice) on screen, comments in chat rooms or physical presence in any of the community areas of the venue where activities related to the program are occurring.  AZNHA may at it’s sole discretion decide to take photographs and/or capture video (including live-streaming or recording for re-broadcast) of our programs for use by AZNHA in promotional material, in print, electronic and other media, including the AZNHA website. All media captured at the program becomes the property of AZNHA.  By participating in a program, individuals release AZNHA, its officers, agents, and each and all persons involved from any liability connected with the taking, recording, digitizing, or publication and use of interviews, photographs, computer images, video and/or or sound recordings and are also waiving all rights to any claim for payment, royalty or compensation of any kind in connection with any use by AZNHA.  Individuals are also waiving any right to inspect or approve any photo, video, or audio recording taken by AZNHA or the person(s) or entity designated to do so by AZNHA.  Individuals may request a copy of certain media (e.g. a photograph of the individual taken at the program) by contacting an AZNHA Team Member to determine its availability.
